A back pass by Antoine van der Linden was not gathered by keeper Remko Pasveer and he pushed the ball to Toornstra, who took his chance.
Coutinho showed great reflexes to keep out Everton's header as the home side pressed for an equaliser.
The ADO keeper was again involved in the action during another Heracles attack which ended with Willie Overtoom heading the ball into the side-netting.
Coutinho was on hand after the break as he denied Overtoom who shot fiercely from distance, before keeping out a volley from Mark Looms.
Having weathered the Heracles storm the visitors were able to extend their lead in the closing minutes as Toornstra fed Vicento and he netted the clinching goal.
