Jarolim, 33, arrived at the Haute-Savoie club in the summer, but an official statement said he had decided to move on for "personal reasons".
The statement added: "ETG-FC regrets the decision of David and wishes him every success for the rest of his career."
Midfielder Jarolim, who won 29 Czech Republic caps between 2005 and 2009, said when he made the move to Evian that he "really wanted to play in France for sporting reasons".
